چکیده

Originally the main idea was to obtain a stable patella, i.e., to stabilize the "slipping patella". In the past many conditions like patella alta, ligamentous laxity, PF bone hypoplasia, weakness of the quadriceps muscle, genu valgum or genu recurvatum were thought to predispose to patellar instability. For a long period muscle exercises were instituted to strengthen the weak m.vastus medialis and to make vastus lateralis stronger. This pulls the patella laterally, especially during running or jumping, when lateral luxation of the patella occurs. Muscle imbalance as well as anatomical abnormalities are the basis both for patellar instabilities and reasonable surgical procedures were: proximal extensor mechanism realignment, proximal capsular reefing, patellar tendon splitting and its medial transfer. On the other hand bone procedures on the hypoplastic lateral femoral condyle were also performed by Albee, as well as tibial tubercle transfer and trochleoplasty by deepening of the trochlea (Dejour). An understanding of the pathoanatomic basis is the corner stone for

Patellofemoral instability: classification and imaging.

Patellofemoral disorders must be approached through an appropriate process of diagnostic framing, performed using language that is, as far as possible, unequivocal and a validated and organic classification system. At present, the classification proposed by the Lyonnaise school, which fulfills these requirements, is the most complete. This classification divides patellofemoral disorders into th...

Chondral Injury in Patellofemoral Instability

OBJECTIVE Patellofemoral instability is common and affects a predominantly young age group. Chondral injury occurs in up to 95%, and includes osteochondral fractures and loose bodies acutely and secondary degenerative changes in recurrent cases. Biomechanical abnormalities, such as trochlear dysplasia, patella alta, and increased tibial tuberosity-trochlear groove distance, predispose to both r...

Patellofemoral Joint Instability: A Review of Current Concepts

The patellofemoral joint remains the enigma of orthopedics and sports medicine. Patellar dislocation is a common problem in the younger and athletic population and it is more disabling than cruciate ligament injuries. The pathology is often multifactorial and complex with no one factor being the sole defining etiology. The current management of patella dislocation has been linked with poor pati...

Pathophysiology of Lateral Patellar Dislocation

Patellofemoral disorders represent 20–40% of all knee problems and can be one of the most common complaints in sports related injuries. These disorders are a major cause of disability, particularly in females, and in extreme cases may contribute to termination of athlete’s career and could lead to degenerative arthritic changes of the knee joint. For these reasons, disorders and in particular p...

Patellofemoral arthroplasty for symptomatic nonunion after trochlear osteotomy for patellar instability: a case report

INTRODUCTION Patients with patellofemoral instability with trochlear dysplasia may be treated with trochlear osteotomy. CASE PRESENTATION We present a patient with patellofemoral instability treated with trochlear osteotomy. The procedure resulted in nonunion with painful bony impingement and isolated patellofemoral osteoarthritis. Patellofemoral arthroplasty was performed. CONCLUSION Patel...

Medial Patellofemoral Ligament Reconstruction Using the Hamstring Tendon for Patellofemoral Joint Instability in an 81-Year-Old Female

INTRODUCTION Chronic patellofemoral instability occurs mainly in adolescent females and can also be induced by medial patellofemoral ligament (MPFL) injury. There are no case reports of MPFL reconstruction for chronic patellofemoral instability due to MPFL injury in aged populations. CASE PRESENTATION 81-year-old female presented with left knee pain, giving way, and patellar instability while...
